Survey for learner feedback

How do I set up an assessment question to use the colored feedback options from the theme? I want the correct choices to have the green outline/checkmark and the incorrect choices to have the red outline/X, but not reveal the correct answer because we allow multiple attempts on our assessments. Currently, the only feedback I can get it text based feedback. Nothing I change on a Test question allows the theme colors to work properly.  

How would you suggest doing a forced ranking of 5 items? I thought to use the number input boxes but not sure how to ensure that 1 unique number gets put in each box--instead of all 1's etc. We also need to track these rank/numbers per item via xAPI.  

Hi,  This might be a long shot, but I'm curious if DK has the ability to give my any data or feedback for a course.  Once I publish a scorm package and upload it to the LMS, is there any way of getting some analystics or data around when the learner stoped, jumped out of the course, number of correct or incorrect attempts of a specific question, time on a page etc? I have a test where learners will need to indicate if they Always, Often, Sometimes, Rarely, or Never do an action.  I want to give different weights to the different answers.  For example, for question 1: Always=10pts, Often=8pts, Sometimes=5pts, Rarely=2pts and Never=0pts For statement/question 2 it may be something like this: Always=0pts, Often=2pts, Sometimes=5pts, Rarely=8pts and Never=10pts

Hi, I have a form on the first page of my course to capture learner data (name, email etc). I'd like the "Submit" button on the page to send the data as an XAPI statement to my LRS so I can record that the user has accessed the course. I would also love for the variables (First Name, Last Name) to replace the userID sent for the remainder of the interactions for the course. Hello there - we've been getting some complaints about the number of clicks it takes to get through a Practice Question. Is there a setting that allows the feedback to be instant (eliminating a submit button, which to me would not be logical) - or to automatically advance the page after the feedback popup is closed? Tried adding an action on the default submit to go to the next page... would be great if there were something that would trigger that on popup close. I think we could customize so...
